How to Form a Team
Team Leader resources are designed to help applicants who are interested in attending a Certificate Program form a team. Applicants are strongly encouraged to apply as a team of up to seven individuals from the same jurisdiction, however smaller teams may be appropriate depending on the scope and focus of a particular jurisdiction. Teams should be comprised of applicants with demonstrated readiness for implementing reforms, especially efforts that engage leaders in other systems of care.
